History
• Painful neck movements (cervical spondylosis)
• Fasciculations, weakness (motor neuron disease)
• Associated sensory loss (syringomyelia)
• Family history (Charcot–Marie–Tooth disease)
• Ascending muscle weakness (Guillain–Barré syndrome)
• Trauma to upper limbs (bilateral median and ulnar nerve lesions).
