Oral vs injectable semaglutide: 8 clinical differences that matter

Semaglutide is now the only GLP-1 receptor agonist available in both an injectable and an oral formulation, and since the oral Wegovy tablet was approved for weight management, prescribers face a genuine choice between two routes for the same molecule. The two are not interchangeable. They differ in pharmacokinetics, dosing, administration burden, tolerability and practical fit, and those differences shape who each one suits. Here are the eight that matter most in practice.

1. Bioavailability: roughly 1% oral versus 94% injectable

The defining difference is bioavailability. Subcutaneous semaglutide enters the circulation almost completely, with bioavailability around 94%, because it bypasses the gut entirely. Oral semaglutide has a bioavailability of approximately 1%, as stomach acid and digestive enzymes degrade the peptide before absorption. Everything else about the oral formulation, the high dose, the strict administration rules, the variability, follows from this single pharmacological fact.

Subcutaneous semaglutide reaches the bloodstream almost completely; the oral route delivers a fraction, which dictates the much higher tablet dose.

2. The SNAC absorption mechanism is unique to the tablet

The oral tablet absorbs at all only because of an added excipient, SNAC (sodium N-[8-(2-hydroxybenzoyl)amino]caprylate). SNAC transiently raises the local pH around the dissolving tablet in the stomach, creating a protective microenvironment that shields semaglutide from acid degradation long enough for a portion to cross the gastric mucosa. The injectable contains no SNAC. This mechanism is also why absorption is so sensitive to gastric conditions: anything that disturbs that local environment, food, fluid volume, co-administered drugs, reduces uptake.

3. Dose: 25mg once daily versus 2.4mg once weekly

Because so little oral semaglutide is absorbed, the tablet is dosed at a maintenance 25mg once daily, compared with the injectable maintenance dose of 2.4mg once weekly. The numbers cannot be compared directly: they refer to very different amounts reaching the bloodstream. The roughly tenfold higher oral dose is simply compensating for the absorption barrier, not delivering more drug systemically. Both follow a gradual titration to limit gastrointestinal adverse effects, the tablet escalating from 1.5mg over about 12 weeks.

4. Administration: a strict fasting window versus none

The oral tablet imposes an administration burden the injection does not. It must be taken on waking, on an empty stomach, with no more than around 120ml of plain water, followed by a 30-minute wait before any food, other fluids or oral medications. Deviations meaningfully reduce absorption from an already low baseline. The weekly injection has no food or timing restrictions and can be administered at any time of day, making adherence conceptually simpler for many patients.

5. Efficacy: broadly comparable at the high oral dose

At the new weight-management dose, oral semaglutide approaches the injectable on efficacy. In OASIS 4, oral semaglutide 25mg produced around 14% mean weight loss at 64 weeks, against approximately 14.9% for injectable 2.4mg in STEP 1. These are separate trials rather than a head-to-head comparison, so the figures are indicative, but they place the high-dose tablet in a similar range. This is a notable shift from the older 14mg oral diabetes dose (Rybelsus), which delivered materially less weight loss.

At the 25mg weight-management dose, oral semaglutide lands close to the injectable, unlike the lower diabetes-dose tablet.

6. Plasma stability and tolerability

The two routes differ in pharmacokinetic stability and discontinuation risk. The low and variable absorption of the oral route produces more variable plasma concentrations than the steady exposure of weekly injection. A meta-analysis in type 2 diabetes also found patients on oral semaglutide had a significantly higher risk of treatment discontinuation due to side effects than those on the subcutaneous form. Tolerability is broadly similar in class terms, nausea, constipation and other gastrointestinal effects, but the daily oral exposure pattern and adherence demands appear to translate into more dropouts in some cohorts.

7. Drug interactions and the empty-stomach rule

The oral tablet’s absorption rules create interaction considerations the injection avoids. Because other oral medications taken within the fasting window impair semaglutide absorption, and because semaglutide delays gastric emptying, co-administration timing matters. In a drug-interaction study, the semaglutide tablet increased levothyroxine exposure by around 33%, warranting monitoring. For polypharmacy patients, particularly those on narrow-therapeutic-index drugs or thyroid replacement first thing in the morning, the oral route adds a scheduling layer the weekly injection does not.

8. Storage and practicality

The formulations also differ in storage and portability. The injectable pen requires refrigeration before first use and careful temperature handling, whereas the tablet is stored at room temperature and needs no cold chain, which simplifies travel and distribution. Set against that, the tablet demands daily dosing and a consistent morning routine, while the injection is a single weekly event. The practical trade-off is convenience of storage and needle avoidance versus simplicity of a once-weekly, timing-free dose.
