Chapter 14 Intrauterine Pressure Catheter Placement



Common indication


When it is not possible to fully assess the pressure of uterine contractions with external monitors, an intrauterine pressure catheter (IUPC) can be used. The IUPC provides a more accurate assessment of uterine contractions.



Equipment


The IUPC is packaged with the guide in a sterile kit (Figure 14-1).

Key steps



1. Cervical exam: Explain the procedure to the patient, and gently examine the cervix. Confirm that the cervix is dilated at least 2 cm and that the membranes are ruptured. The artificial rupture of the membranes will be necessary if they are still intact. Set the baseline to “0.” If amnioinfusion is planned, flush the amnioport with infusion solution (Figure 14-2).

2. Place fingers on presenting part: Pass the examining fingers through the introitus and cervix, and place the fingers against the fetal presenting part. If the location of the placenta is known, place the catheter on the opposite side of the uterus (Figure 14-3).
