CASE 17
The parents brought their 4-week-old son to the pediatrician complaining that he suffered episodes of projectile vomiting. It was learned from the history that the vomitus was nonbilious and that the infant had a voracious appetite between vomiting episodes. The physical examination revealed peristaltic waves in the epigastrium, and a palpable mass was detected in this area as well. Imaging studies (ultrasound and radiologic) revealed a thickened pyloric sphincter (Fig. 3-14). The infant was diagnosed with congenital hypertrophic pyloric stenosis and was treated by pyloromyotomy.
