A 55-year-old man visits his primary care physician complaining of burning abdominal pain that begins shortly after eating.


The patient has experienced pain associated with eating for the past few months. Sometimes the pain diminishes after eating and recurs 2 hours later. In the past, the pain had been relieved by antacids but not by nonsteroidal anti-inflammatory drugs (NSAIDs). The patient has smoked one pack of cigarettes a day for 40 years and drinks 4 cups of coffee a day. He reports an 8-pound weight loss over the past 3 months without intentional diet or exercise.
