Dilated pore of Winer (DPW)
Enlarged follicular structure with acanthotic squamous lining and central keratin-filled space
Etiology/Pathogenesis
May be related to preceding inflammatory cystic acne in some cases
Solar damage also implicated as most cases occur in elderly
Clinical Issues
Head and neck region most common
Excellent, completely benign (likely nonneoplastic) lesion
Microscopic Pathology
Enlarged, dilated follicular structure
Infundibular-type keratinization with a granular layer
Squamous epithelial lining shows acanthosis and finger-like projections into surrounding dermis
 Dilated pore of Winer (DPW) at low-power examination shows a follicular structure with a dilated opening filled with keratin debris. This lesion shows focal rupture and associated inflammation  . |
 Intermediate magnification of a DPW shows the acanthotic squamous lining epithelium with focal fingerlike projections  and mild basilar hyperpigmentation  . |
